Duty of Disclosure
You should answer the questions asked by Us honestly, fully, and accurately. Failure to give answers that are full and accurate may result in Your policy being voided, a claim not being paid or reduced, or the terms of the policy being changed. You are advised to take reasonable care not to make any misrepresentation when answering any questions asked by Us. In addition to answering the questions in this proposal, You are also required to take reasonable care to tell us any matter which You know to be relevant to Our decision on whether to accept the risk or not and the rates and terms to be applied. You must inform us if there is any change to the information given in this proposal after You submitted this proposal but before the contract is entered into or varied. lf You do not understand Your duty as stated above or if You need any further explanation, You can contact Us.

1. I have read and understood the Product Disclosure Sheet(s) provided to me and I understand that the Product Disclosure Sheet is a summary of the main features of Tokio Marine Life’s product(s). The information contained in the Product Disclosure Sheets is intended for illustration purpose and does not constitute a contract of insurance. I shall refer to the policy document for full details of the terms and conditions of the respective products, including those outlined in the attachment(s) hereto.
2. I have read and understood the Duty of Disclosure and I declare that all details I have entered in this application are true, correct and complete and I have not withheld any important or relevant information to this application. In the event of non-disclosure during underwriting stage, Tokio Marine Life Insurance Malaysia Bhd. (TMLM) reserves the right to void the policy, reject or reduce a claim and/or change or vary the terms after system validation at TMLM.
3. I have read and understood TMLM's Privacy Policy and Data Protection Policy and agree to provide my consent for my personal data to be used for the purposes stated therein.

TokioMarine-CriticalDefender

1. I am aware that TokioMarine-CriticalDefender covers Kidney Failure and Cancer.
2. I am aware that TokioMarine-CriticalDefender does not cover Death, Total and Permanent Disability, Hospitalization, and Accidents. I have read the Product Disclosure Sheet and understood the full list of exclusions, terms and conditions.
